Re: Questionnaire for an International study on respiratory health.
My name is Philippa Ellwood. I, along with Dr Rebecca Potts and Judy Rowden are staff members in the Department of Paediatrics: Child and Youth Health at The University of Auckland. We are involved in an International Study on respiratory health. You are invited to take part in an important worldwide health study and we would appreciate your assistance. This is an online questionnaire about the health and the environment of you and your 6 or 7 year old child. Your child’s school Board of Trustees and Principal have given permission for this study to take part.
You will be sent a link to a secure online system so that the questionnaires can be completed and submitted online. If your child lives with two parents (or guardians), we would be grateful if each of you could complete an ADULT questionnaire. If the child lives with one parent (or guardian), we expect that only one copy of the ADULT questionnaire will be completed. If one parent/guardian could also complete the questionnaire for your child, we would be grateful.
You are under no obligation to be involved. You are not being asked to provide us with your name or address on the questionnaires, however we will be using a coding system to enable us to link the Adult data to that of the children. No personal identifying information will be entered into the computer. Withdrawal from the study, therefore, will not be possible as the data is collected anonymously and it will not be possible to match the data up with an individual participant. All data will be kept on a password protected University of Auckland computer and backed up by the University’s server. If you would like to contact one of us to discuss this research before you complete the questionnaires, you are more than welcome to do so using the contact numbers below. The results of the worldwide study will be published by country or region. No individual results will be made available. A copy of the collective results from your child’s school will be sent to the Principal and you are welcome to access these.
The Principal of your child’s school has given an assurance that you and your child’s participation or non-participation will have no effect on your child’s relationship or grades within the school.
If you would like to participate in this study, we would be grateful. This study will involve over 50 countries and has received ethical approval from the University of Auckland Human Participants Ethics Committee.
